After The patient applied for medical advice and treatment at the Pediatric Cardiac Surgery Center. His medical file was studied by Professor Suleiman Ozkan, Director of the Pediatric Cardiac Surgery Center and Medical Director Dr. Hasan Kasimoglu. The Board of Physicians decided to perform open-heart surgery as a complementary operation for congenital heart malformation   in a child .  (The child underwent two open heart surgeries before, during which the chest was completely opened, and it is his third operation.)

The family was informed of all the details and that the operation was necessary and urgent.  After the patient arrived at the hospital, all diagnostic tests were performed again, and the medical specialties were consulted :

• Clinical examination of the child in the Department of Cardiology and tests: ECG- echocardiography- angiography- chest x-ray.
• Clinical examination in the Anesthesia Department, and blood tests- Covid test
• Clinical examination in the Department of cardiovascular Surgery.

After the approval of the child’s family on the open heart surgery, we performed operation of Hypoplastic Right Heart Fontan Surgery, thanks to God and the experience of the professor. The operation was successfully completed, and now the patient is in good health.

Information about Hypoplastic Right Heart Fontan Surgery

On 12.24.2021 the child was admitted to the operation, under general anesthesia and intravenous catheters were done in to measure the blood vessels and the superior vena cava.

Diagnostic catheterization was done and under general anesthesia, the superior vena cava (SVC) was entered.

The right pulmonary vein was carrying blood from the open lung but there was mild stenosis in the right pulmonary artery.

Pulmonary arterial pressure was measured at 15 mmHg, and the operation was stably applied.

Decision: The child’s condition is stable and appropriate to do total cavopulmonary connection (TCPC) and complete  in the Fontan process inside  or outside the heart, and the fenestration is not clear.

The surgeon was entered under the Kardiyopulmoner Bypass (KPB) and without interruption of cardiac application a total cavopulmonary connection (TCPC) was performed outside the cardiac lumen and 18 mmHg was measured.

The fenestrated fontan process used a high-quality PTFE synthetic graft that is a fluoropolymer made of Polytetrafluoroethylene (PTFE). A 5-millimeter graft was used. It was applied between it and the right atrium vein (right atrium bridge and PTFE atresia).

According to the child’s condition, the Blalock-Taussig shunt operation was performed and closed it to transferred to the Glen operation and the measurements were made:

Pulmonary arterial pressure was measured at 15 mmHg. At the beginning of the operation, it was 15 mmHg, and at the end of the operation in the intensive care unit was 15 mmHg.
