Chronic pain is a pain that lasts more than 6 months (in some cases more than 3 months) and is more than an alarm system.
Chronic pain is not a precursor to the disease, but rather the problem itself. A person suffering from chronic pain gets into a vicious cycle, the patient loses power and potency. loses his productivity and activity.
This often leads to depression. Depression makes a person more sensitive, lowers pain threshold and makes the pain more intense. This condition is a vicious cycle of complete pain. Chronic pain is not a symptom of the disease, but rather the disease itself.
Chronic pain is a serious problem that affects not only the patient who suffers, but also the patient’s immediate environment. The social and economic loss caused by chronic pain should not be ignored. In an American study, it is estimated that 700 million workdays and $ 60 million losses occur each year due to chronic pain. Pain therapy methods
Since the advent of the medical profession, pain relief has been one of the primary goals for doctors. The use of pain drug in modern medicine has an important place in therapy. However, the important point here is that pain killers should not be used in an unsupervised and irregular manner and that the World Health Organization’s principles of pain relief should be followed. These principles define the manner in which pain relievers are used, the dosage, when pain medication will appear, and ways to deal with side effects that may be encountered while substance abuse. Studies have shown that more than 90% of all types of pain can be stopped with the right pain medications. In cases where pain relievers are not effective and adequate, physical therapy methods, surgical procedures or interventional pain therapy methods are applied according to the source of pain. At this point, the correct approach is to determine the most appropriate treatment method for the patient and to ensure that the patient accesses the appropriate treatment without wasting time.
The main methods used to treat pain in the pain clinics are pharmacological and interventional pain therapy methods. Interventional pain management methods are the primary source for the development and management of chronic pain during anesthesiology.
These interventions, which are defined as minimally invasive methods, aim providing easy and effective pain relief for difficult-to-treat pain without the need to inconvenience the patient. These methods are mainly a nervous block.
There are different types of nerve fibers in the body. Some nerves are responsible for muscle movement, while others are responsible for pain transmission. These pain nerves are our field of interest. For example, trigeminal neuralgia, which appears with severe pain on the face, the occlusion applied to the trigeminal nerve guarantees long-term (3-8 years) pain relief.

General information about interventional methods
Non-surgical pain management techniques called interventional methods are usually performed using electrodes such as a needle. Except for a few of them it is performed without creating an incision in the body. The operations are carried out in the operating room environment, according to the rules of sterilization and disposable materials.
All materials used for the patient are disposed of at the end of the operation according to the principles of biomedical waste removal and completely new and unused instruments are opened to a new patient.
Patients are given sedatives and pain medications to avoid discomfort during the procedure. This is not a general anesthetic. Most of the time, the patient is conscious, but does not remember any discomfort after the operation.
Patients are monitored during the interventions by being connected to a monitor that displays vital functions (blood pressure, heart rate, blood oxygen rate, etc.).
All interventions are performed using the radiographic method, that is, by observing the area in which the intervention is performed. This is necessary for the success of the procedure and to eliminate side effects.
Patients are monitored for several hours after the procedure. After some attempts, they are kept in hospital for one night and sent home the next day.
Interventional pain management methods applied in the hospital:
Epidural injection: In hernias originating in the parts of the waist, neck and back in the spine, pain is treated by eliminating pressure by injecting various drugs into the spinal cord, where the hernia leads to pressure by creating nerves.
Radiofrequency thermal coagulation: It is a method of disrupting the nerves that transmit pain by generating heat with the help of high-frequency radio waves. It is worth noting that this method is that nerves with conduction inactivated are sensory nerves that transmit pain, not motor nerves. Therefore, the patient does not have any restriction of movement after the operation, only the pain disappears. Major neurological diseases (trigeminal neuralgia), shingles, cancer pain, calcification of the waist and neck joints (facet removal) are the most important painful diseases.
Epidural lysis: The adhesions that form around the nerves in the operation area after operations for a lumbar or neck hernia can cause severe pain.
The formation of these adhesions is not related to the error of the surgeon, but is completely related to the patient’s body. In this case, a special probe such as a guitar string is placed in the spinal canal and special medications are given for 2-3 days to eliminate the adhesions.
Nerve blocks: in cancer pain, neuralgia, persistent hiccups, block transmission of pain and drugs to block nerves.
Sympathetic blocks: The sympathetic nervous system is a system that controls involuntary functions in the human body and plays an important role in pain transmission. When the proper conduction of the sympathetic nerve is interrupted, the patient’s pain disappears, but no unwanted change in other functions occurs.
In severe types of pain called neuropathic pain and cancer pain, various sympathetic blocks are applied.
Morphine pumps: It is the process of relieving pain by inserting morphine into the spinal canal through probes connected to a special pump placed under the skin. It is mostly applied in pain due to cancer and vascular occlusion.
In addition to effective pain relief, an important advantage of this method is the use of low-dose medications.
Trigger point injections: Trigger points occur in muscle tissue and cause pain to spread. These points cause head, neck, shoulder, back pain and lower back pain depending on their location. These types of pain are treated with drug injections to these exciting points. Nucleoplasty: Lumbar hernia can be treated without surgery. It is one of the newest in the medical world. In this operation, the patient’s herniated disk is inserted through an electrode. A special current is inserted into the disc to reduce disc pressure and the herniation to retract.
Diagnostic methods: With some painful patients, it can be difficult to determine the cause of the pain. In these cases, it is possible to identify the nerves in the body that arise from the pain by interventional methods. In cases where the cause of the pain is not understood, the source of the pain can be determined using drugs provided by probes placed on the potential nervous system using imaging methods.
Quality of life
One of the most important damages of chronic pain is a deterioration in the quality of life. Quality of life, people’s perception of their situations in relation to their expectations, standards, and concerns about the culture and value systems they live in in society.
Concept of physical and mental health; Family and work life, economic status, level of education, ability to perform independent functions, self-confidence, success in social relations and self-satisfaction with the state are determined.

Pain is a personal feeling, unique to the individual. The best way to assess pain is to rely on the patient’s expression and consider the patient’s pain very seriously. Because it is the patient who suffers from pain and this pain leads to a deterioration of the patient’s quality of life.
